So You're Leaving Benchmark... What Next?

DU AND BENCHMARK PROPERTY

Please return the following property to the appropriate locations:

  • DU ID CARD, TOLL TAG, and NAMETAG: Please return these items to the HR office.
  • UNIFORMS: Please ensure all uniforms are turned in on your last day.
  • LAPTOP COMPUTER, iPAD, or CELL PHONE: Please make arrangements to turn these into Benchmark's IT Department.
  • OTHER DU PROPERTY: (I.e.: radios, itouch, earpieces, cash drawer keys, etc.): Please turn these items into your leader.

Failure to turn in DU property on your last day may result in deductions for these items on your final paycheck.

MEDICAL, DENTAL AND VISION COVERAGE

Your medical, dental and vision plan coverage will end on the last day of the month in which you were terminated. For example, if your last day of employment is April 14, your coverage will end April 30.

COBRA COVERAGE - CONTINUATION OF BENEFITS

You will receive Consolidated Omnibus Budget Reconciliation Act (COBRA) enrollment information 30-40 days following the end of your employment. The information will include premium costs and enrollment deadlines. If you do not receive your COBRA information within the above-mentioned time frame, please contact our COBRA Benefits Administrator, WEX Benefits, at 866-451-3399.

LIFE INSURANCE COVERAGE, ACCIDENTAL DEATH & DISMEMBERMENT (AD&D)

Basic and Voluntary Life and AD&D coverage is portable, you can apply to continue these benefits at your own cost.  Please contact our Life Insurance company, Securian, at 1-800-843-8358.  This insurance will end on your last day of employment.

SHORT TERM DISABILITY (STD)

If you have STD, your benefits will end on your last day of employment with Benchmark. STD coverage is not portable.

LONG TERM DISABILITY (LTD)

Long Term Disability coverage will end on your last day of employment, but you can apply to continue this benefit by contacting UNUM’s Conversion Group at 866-220-8460.  Benchmark’s LTD Policy Number is 205842.

ACCIDENT, CRITICAL ILLNESS, AND HOSPITAL INDEMNITY INSURANCE

If you have supplemental benefits through Voya, such as accident insurance, critical illness insurance, and hospital indemnity insurance, your benefits will end on your last day of the month after you leave benchmark. Supplemental coverage is not portable.

401K

Benchmark’s 401k administrator, Fidelity, will forward information to your home address outlining the various options you have with regards to taking a distribution from the plan or continuing payments for existing loans. If you do not receive this information, please contact netbenefits.com.

FLEX SPENDING (FSA)

If you have any funds in your FSA you can make a claim with Discovery for up to 90 days after your termination date to get reimbursed for anything that occurred before your termination date. Any funds left in your FSA after the 90 days will be forfeited. You can contact WEX Benefits at 1-866-451-3399.

W2 TAX FORM

Your W2 will be mailed by January 31 to your address listed in Workday. If your address changes, please update it by calling Human Resources at 682-388-1234.  You will continue to have access to check this on Workday for 1 year.
