Medical Insurance - UnitedHealthcare

Two of Benchmark's four health insurance plan options are traditional HMO United Health Care plans: UnitedHealthcare Navigate and UnitedHealthcare Saver.

The UnitedHealhcare Navigate plan offers in-network coverage only and you must designate your Primary Care Provider at the time of enrollment using your providers' ID number. Please follow the instructions in the drop-down field below to locate your Physicians’ provider ID number.

Medical Insurance - Surest

Two of Benchmark's four health insurance plan options are PPO Surest plans.

Surest is a United Health Care Company with no deductibles or coinsurance that strives for price transparency. Members can check costs and compare options for doctors, treatments, and drugs before making an appointment. Prices for health services that often occur together are grouped into a single copay. You know what's owed in advance and receive a single bill for the services delivered in that one visit.

Express Script logo

Prescription Plan

All four of Benchmark's Health Insurance Plans. Your drug coverage will depend on the medical coverage level you choose. Medications are grouped into tiers which determines your portion of the drug cost.

Maintenance medications should be filled at an Express Script Retail Pharmacy (CVS, Walgreens, Walmart, etc.) or though CVS Caremark's Mail Order 90 Day Supply.

Medical Plan Option Summaries

All full time employees are eligible for coverage on the first of the month after their hire date. All new hires will have 30 days after their hire date to submit benefit elections via workday. If benefits are elected after the benefit start date, the employee will be responsible for making back payments.  A spouse, domestic partner, or child up to the age of 26 can be added to the coverage as a dependent.

Changes can only be made to your benefits during open enrollment or if a qualifying life event occurs.

Qualifying Life Events Include: Loss of Medical Insurance Coverage, Dependent Loss/Gain of Medical Insurance Coverage, Marriage, Divorce, Birth or Adoption of a child.

To change insurance elections mid-year please send document evidence of qualifying life event to dekassel@deloitte.com.

Health Savings Account (HSA)

What is an HSA?

A Health Savings Account is a personal bank account for medical expenses. You are able to deposit pre-tax dollars from your paycheck to your Fidelity HSA to pay for medical services as well as certain over the counter health items. Money that is deposited into the account will accrue growth, just like a 401(k) and the money in the account rolls over from year to year, so you will never lose the money you deposit.

How do I sign up?

You must be enrolled in the United Health Care Navigate Saver Plan. If you do not already have an HSA and would like one, switch to the Navigate Saver Plan during your next open enrollment period.

Flexible Spending Account (FSA)

What is an FSA?

A Flexible Spending Account is similar to an HSA, but has key differences. An FSA is a health care savings account that collects pre-tax dollars from your paycheck to put toward eligible medical expenses. All contributions are decided at the time of enrollment and cannot be changed. If there is any funds in the FSA at the end of the year, the amount remaining will be forfeited. Benchmark's FSA is through Wex Discovery Benefits.

How do I sign up?

You must be enrolled in either of the Surest Plans, or the United Health Care Navigate Plan. If you do not already have an FSA and would like one, switch to an eligible Health Plan and enroll in a FSA during your next open enrollment period.

Supplemental Insurance

Accident, Critical Illness, and Hospital Indemnity Insurance

Benchmark's accident insurance, critical illness insurance, and hospital indemnity insurance plans are each through Voya and can be elected together or separately.

These plans are designed to pay a lump sum benefit directly to the insured if they experience a covered accident, diagnosis of a covered critical illness or a covered hospital stay, regardless of any other coverage.

Wellness Benefit: Each Voya plan includes an annual wellness benefit for the enrolled employee and their covered dependents that will pay $50 per plan you are enrolled in for completing an eligible wellness screening.

Long-Term and Short-Term Disability

Benchmark's Short-Term Disability and Long-Term Disability Insurance plans are through Unum.

Unum will replace 60% of your income if you are unable to work due to accidental injury, illness, or pregnancy. The policy must predate the condition or disability to be eligible for disability benefits (i.e., if you are pregnant before electing your short-term disability coverage, you will not be eligible for disability benefits upon the birth of  the baby).

Short Term Disability: 26 weeks of coverage after a 7-day waiting period with a maximum benefit of $1,200 per week.

Long Term Disability: After a 180 day waiting period, employee will be covered until they reach social security normal retirement age with a maximum benefit of $15,000 a month.

Rates will vary based on demographics. Long Term Disability Coverage will be provided free of charge to all salaried employees and managers.

Life and Accidental Death and Dismemberment Insurance

Benchmark's Life and AD&D Insurance is through Securian.

Benchmark offers a basic life insurance and basic accidental death and dismemberment insurance that is employer paid. Each of these two plans cover the employee for 1x their annual earnings up to $100,000 for hourly employees, and $300,000 for salaried employees and managers.

We also offer supplemental life and accidental death and dismemberment policies. Employees can elect coverage up to 5x their salary with $200,000 guaranteed issue for hourly employees, and $400,000 for salaried employees and managers. Spouses can elect coverage in increments of $25,000 up to $100,000 with $50,000 guaranteed issue not to exceed 100% of the employee's life insurance elections. Children can be covered up to $10,000 for each child.
